Staff Selection Commission: As per the revised formats, form 5 will be for single disability and form 6 for multiple disabilities.
The Staff Selection Commission (SSC) has revised the disability certificate formats in accordance with the Department of Empowerment of Persons with Disabilities (DEPwD). As per the revised format, form 5 will be for single disability and form 6 for multiple disabilities.
The updated formats replace the previous three forms 5, Form 6, and 7, that were used in various notifications issued earlier by the commission.
“It has been decided by the Commission that for all examinations where the Notices were issued after October 16, 2024 and the recruitment process has not been completed, candidates may submit their Disability Certificate in either the revised formats (Form V and Form VI as per the DEPwD Notification dated October 16, 2024) or in the earlier formats,” the official SSC notice reads.
Recently, SSC restored ‘own scribe’ facility for PwBD candidates, with effect for exams notified on or before December 31, following the clarification issued by the DEPwD.

Furthermore, the facility is subject to new compliance conditions. Under the revised norms, the scribe’s age must align with the qualification level required for the exams. For the matic level and Class 12 exam, the scribe should generally not be above 20 years of age; while for the graduate level, the scribe’s age should not exceed 22 years.
Additionally, Aadhaar-based authentication is now mandatory for all scribes registered under the “Own Scribe” facility. If a scribe cannot complete Aadhaar verification at the examination venue, the candidate must either use a scribe arranged by the Commission or take the exam without one.
